In this session, Adrian Zai, MD, a national leader in population health management at Massachusetts General Hospital in Boston, will outline how an academic medical center achieved and sustained clinical outcomes by evaluating and understanding its provider workflow and adjusting the organizational structure to support a team-based strategy. Population health technology, which supports the workflow and infrastructure adjustments will be described, as it addresses data, analytics and decision support areas.
