Adrian Zai, MD, is the clinical director of population informatics at the Massachusetts General Hospital’s Laboratory of Computer Science. He is the lead designer and innovator of TopCare, an information technology platform designed uniquely for population health management with core components that support team-based care and patient-centered management. In 2012, Partners Healthcare selected TopCare as their enterprise Population Health Management solution. In collaboration with SRG-Technology, TopCare has been deployed at the Massachusetts General Hospital and Brigham and Women’s Hospital.

In this session, Adrian Zai, MD, a national leader in population health management at Massachusetts General Hospital in Boston, will outline how an academic medical center achieved and sustained clinical outcomes by evaluating and understanding its provider workflow and adjusting the organizational structure to support a team-based strategy. Population health technology, which supports the workflow and infrastructure adjustments will described, as it addresses data, analytics and decision support areas.
