POP HEALTH FORUM: A HIMSS EVENT

Dr. “Drew” Shah is a Medical School graduate from the University of Medicine & Dentistry of New Jersey and completed his residency program at Hoboken University Medical Center. Dr. Shah earned his Bachelor of Medicine/Bachelor of Surgery from Municipal Medical College in Gujarat, India and is Board Certified in Family Medicine.
Care management and team-based care delivery transformation are key factors for any population health strategy. This session explores how an independent physician group, Compass Medical, used data-driven insights and dedicated care management teams to transform its regional ecosystem. By leveraging big data and patient engagement, Compass developed a strong care management team and created a 360-degree view of patient care across healthcare domains. Team-based care has paved the way both for an improved patient experience and a reduction in provider burnout.
One of the most valuable ways to learn is through interaction with your peers. Our “Speed Networking” session will provide an opportunity to meet speakers and attendees who have similar pop health challenges and discuss solutions to those potential obstacles. Mingle, share and learn in this unique, rapid fire and interactive environment.
