Investing in Non-Billable Services to Address Social Determinants of Health

October 2, 2017
2:50pm - 3:20pm
Grand Ballroom

New Jersey-based Shore Quality Partners, a clinically integrated network that includes 240 physicians, over the past few years sought to better manage the health of their patients, including 15,000 recently laid-off workers from Atlantic City, who found themselves without healthcare.

In this session, attendees will learn how Shore Quality used data and analytics, non-traditional care activities – free counseling and educational resources, for example – and established new clinical care protocols to better manage psychosocial aspects of patient health.

Overall, the 1,250 non-billable encounters had a remarkable impact on overall costs. This included decreasing spending by high-ED utilization patients by $1,600 per member per month and inpatient utilization rates by 15%.

Key takeaways will address:

  • The impact of social and behavioral factors on the health risk of a population.
  • A holistic approach to pop health, including strategies to manage social factors for high-risk patients.
  • The clinical & financial impact of non-billable services in addressing social determinants of health in value-based care.
