Today’s Patient Deserves Better - Removing the Barriers to Better Health

Successful population health management requires a broad approach to care that includes helping patients manage social factors outside the clinician’s office: from air quality and sedentary lifestyles to food insecurity, transportation, and even social isolation.

This is the core lesson Humana learned from its Bold Goal initiative, a program designed to help communities it serves become 20 percent healthier by 2020. The company is collaborating with seven communities across Louisiana, Florida, Tennessee, Texas and Kentucky to make healthcare easier and thus create more “Healthy Days,” as measured by the CDC’s survey tool for benchmarking community health.

In this session, attendees will hear how through its Bold Goal program Humana discovered that to improve community health requires local partnerships, clinical interventions and engagement strategies to increase resources for patients that lead to behavior change.
