Dr. Koslow earned his medical degree in 1997 from the Medical College of Pennsylvania (now Drexel University College of Medicine). In 2001, he completed his emergency medicine residency at Albert Einstein Medical Center and was chief resident. Dr. Koslow is board certified in emergency medicine. He also graduated from Boston College Law School in 2009 with honors, and serves as chairman of the national Medical Legal Committee of the American College of Emergency Physicians.
Care management and value-based delivery transformation are key to successful population health outcomes. In this case study, Compass Medical, an independent physician group, shows how it leveraged data-driven insights and dedicated care management teams to mature its value profiles and meaningful predictions framework to drive better outcomes. This transformation allowed Compass to redesign care delivery and enhance provider & patient experience. What’s more, addressing provider burnout and empowering care teams through this approach has accelerated the group’s path forward.
