POP HEALTH FORUM: A HIMSS EVENT
Chicago, IL - October 2 - 3, 2017
Tiffany McCauley, MSN, RN serves as a Nursing Executive with Elsevier’s Clinical Solutions. In this role she collaborates with healthcare systems, academic centers and physician practices to reduce variation in care, standardizing practice and improving patient outcomes through the application of evidence-based decisions at the point of care.
Prior to joining Elsevier, Tiffany led the development of nursing and patient education programs at a number of leading community-based healthcare organizations. Her focus has been on improving the patient experience by leading change initiatives related to clinical workflow, patient services and standardizing patient education between the physician practices and the hospital. Tiffany believes this is a key component in improving the healthcare experience for patients.
As value-based care continues to replace the fee-for-service, providers are under increased scrutiny to improve the health of their patient population. While past history and demographics play an important factor in determining risk, a patient’s social determinants, if not considered, can negatively impact outcomes. Elsevier’s Tiffany McCauley will discuss how socio-economic factors can add an important dimension in coordinating longitudinal care and aligning your resources for maximum benefit.
