In 2010, Northwell Health, a 21-hospital health system in the New York metropolitan area, understood the importance of having the right foundation for its IT infrastructure. The organizations started building enterprise HIE, DWH, and care tools. By 2017, it had most of the tools needed and shifted gears into scalable utilization and adjustment.
As Northwell transitions from reactive care to proactive care, it uses its vision of 2020 to identify essential technological enablers and set execution tactics accordingly. Today, these enablers include improved analytics capabilities, incorporating and disseminating an integrated care plan, deploying a longitudinal and holistic single patient record and online communication tools to foster patient self-management and clinical decision support for the population it serves.
- Communication, interoperability and infrastructure are three elements to invest in.
- Disruption and innovation will rely on these elements as supporting tools for outcome improvement strategy.
- Implementation plan affects any solution scalability.
